In particular, I would look at one of Bellovin's &lt;a href="http://www.cs.columbia.edu/~smb/blog/2008-02/2008-02-07.html"&gt;recent posts&lt;/a&gt;, explaining that for at least one of the failures: "&lt;i&gt;an abandoned ship anchor&lt;/i&gt;" found nearby "&lt;i&gt;categorically that that was the cause of that failure&lt;/i&gt;"</description>
